Birthday Party Form

•COVID-19 NOTICE: WE ARE CURRENTLY DOING ONLINE PARTIES ONLY. PLEASE DO NOT FILL OUT THE FORM BUT EMAIL FRANK@TODDLETUNES.ORG REGARDING YOUR DAY, TIME AND WHAT TYPE OF EVENT YOU'RE HAVING. 

 Please take a moment and fill out the form. ALL fields are required or your party cannot be processed. 

• We must receive a form to know whether or not we can accommodate your party.
• The form does not obligate you to book the party.
• A one month reservation is suggested for all Birthday Parties and Events.
• Prices for Birthday Parties vary depending on the number of children invited (not rsvp'd).
• We will confirm within 3-5 days.
• Parties are paid in advance.
• Please adjust your email options to accept emails from matthew@toddletunes.org.
• There are NO REFUNDS for birthday parties but we can reschedule you or offer classes for the money spent.
